The land mass in the south east looks a bit realistic. Is there a medical reason for the "double peninsula?"

Your mountains look good.

The tiny land bridge in the north west sticks out. It's just odd, and I think that you'd do well to remove it and just note that there is a shallow land bridge there during low tide. If there's a fiction fact reason to it, please let me know.

The "double peninsula", if you mean the north side of the island, once has a gentler transition to the coast, until a landslide caused a significant section of the island to fall into the sea.

As for the land bridge, I'm not sure that there's really a reason for it. There's not enough current there to wash it away. I suppose I might replace it with a significant man made bridge. The Homraji live in the region and are more than capable of an engineering feat of that scale.

The squid is located where it is for lore reasons. There is an apocalyptic cult with some terrorist like deeds to their name who reside there. They are attempting to bring about a flood that will end the world. Part of doing so is sabotaging the deffenses of foreign cities when the Walking Tide / the House in the Sea comes ashore (think zombies mixed with octopus lobsters, coming ashore by the thousands unpredictably, marching inland feasting on men and livestock as they go.) I thought the squid was a good enough representation of the llongoye, a whale sized Nautilus like creature thought to be used as transportation by the House in the Sea.

Maybe try a different design that wraps around that island. Just so it looks like it is supposed to be there rather than placed down randomly.
