We talked about this in my audit class, apparently it's a pretty common thing to do.

Also sending companies stuff that they didn't order, but will pay for anyway.

If a company doesn't have controls to match invoices with orders, it will be paid pretty much 100% of the time. Any serious company will have such controls by now, I imagine.

Depends on how many "diversity hires" a company has. As has been already shown, Google must have achieved 100% diversity in their accounting department.
