Fear extinction does not work on true PTSD. It may appear to work, because PTSD is not a true diagnosis but a cluster of symptoms. Physiologically based PTSD is what most people think of when PTSD talked about. Psychologists and some pyschiatrist with conflicts of interest on real science keep the waters muddy around PTSD.
It can be proved that fear extinction does not work on genuine PTSD and has been proved. In fact it is the defining characteristic of PTSD. Everything else is just treatable anxiety disorders. If it can be "cured" then it was never PTSD to begin with.
PTSD results from an irreversible brain trauma where something has caused the brain to fail to protect itself with cortisol. From there, a cascade permanently damages the HPA. There are distinct characteristics that can be examined only during autopsies, but they are irrefutable.
PTSD can be managed, but that's about it.
