I can definitely see where you're coming from. Correct me if I am wrong in my assumptions but I am guessing your reasoning is as follows:

  • Police acted unprofessionally, regardless of legality, on scene
  • Corrections officers did not use information correctly to ensure safety while detained
  • Due to lack of proper supervision, the blame should fall on the corrections system

Am I correct?

Yes. Failure to protect a suicidal woman, if she did admit suicidal tendencies, is a failure that should carry weight, and termination.

Unlawful detention and false arrest should not only get the initial contact LEO fired, but jailed.

This changes nothing.
