I'm not sure why you think I'm wrong. If you don't believe that preventing the annihilation of a major population center is enough of a deterrent, than I guess there's nothing more to be said.
It may not be the only reason but you need to read the theories behind modern warfare if you don't think it's a huge issue.

Here's why you're wrong. Assuming what you say is true there is no reason NK wouldn't do what you're saying after they make their first nuclear strike.... especially since they're GOING to get attacked if they do that.
Waiting doesn't do anything except give them a chance to prepare and have a stronger alpha strike. There are no other possibilities.

Obviously they'll get annihilated if they attacked first. Kim is a madman but he isn't suicidal.
Him and his father and grandfather are happy to keep the peace with annual tantrums to get aid.
A first strike would require a buildup of troops and large deployment of naval forces. The NKs would see this buildup and if they honestly feel an invasion was imminent, they'd attack like a cornered animal.
The small American garrison and SK troops would be overran before a successful counteroffensive could be launched.
Mutually Assured Destruction has worked as a policy for the last few decades. Russians and Americans. SK and NK.
