Prior to Roosevelt social protection was organised at a community level by fraternal societies, mostly along professional and racial lines. So if you were a WASP bricklayer for example, you'd join your local chapter of the "Ancient and accepted order of the brickies" or soemthing. You'd pay in every month and in return you'd get medical coverage (lodge practice), life insurance, income insurance etc. If you were sick, injured or killed, your family would be taken care of. And because the people paying for your convalesence were your friends and neighbours there was a strong social pressure not to be a sponger. Also, if someone did refuse to work they could be kicked out (unlike social welfare).
The doctors hated this because it meant they had to negotiate with tradesmen, which was beneath them, and the fraternal societies were very good at driving costs down so they couldn't charge insane prices. The solution was to agitate for professional medical associations to be made licencing bodies, which could control the numbers of doctors to keep costs high, and outlaw lodge practice to force everyone to negotiate individually.

He hated the fraternal organisations because they were a roadblock to his vision of a melting pot where all the races in America were mixed into one and no one cared if someone (just to pluck an example out of the air) happened to be a cryptojew. So the fraternal organisations had to go and they were replaced by new state welfare institutions. It's the same thing Bismarck tried in Germany to bind the peasants to the Prussian state, except with more race mixing.
