How would this happen in the first place? Is our browsing history saved at the ISP level? Lastly, does a VPN do anything to mitigate this?

It's logged at the ISP so a VPN would prevent this. If you trust the vpn provider

You type shit into a search engine. They log that shit. Combined with browser fingerprinting, IP addresses, and VPN logs, it's all tied neatly to you. Are you logged into Google? Bing? Facebook? Twitter? as you search for shit? Guess what.

ISP's and VPN's in the US are allowed to collect and sell your browsing data as far as i'm aware.
