It's easy. Apply a finger to the painful area and if they flinch in pain they might be legitimate.

If it were that easy, we wouldn't have any sort of problems at all. What you're thinking of is acute pain, which many people just learn to avoid triggering. Chronic pain, by definition, is a constant condition; there's no need for the area in question to be more susceptible to acute pain than normal(especially since it's perfectly possible for the actual damage to lie elsewhere).
