The outer dimensions are identical between them. The pressure is the real difference.

Extremely stupid question; would one be able to use these interchangably, since they appear to have identical dimensions?

In other words, is a rifle chambered for 223 able to fire 5.56 and vice versa?

Short answer, yes. A barrel rated for 5.56 can shoot either. I'm sure it works the other way around however if it's only rated for .223 you will damage the barrel rather quickly shooting 5.56 rounds out of it. A lot of the ar-15 have a barrel rated for 5.56 so they can fire either.
