There should be a penalty right off the bat. If the victim (that is, the falsely accused) has any interaction with the police, there should automatically be a penalty for the accuser.

Recant within a few days - no harm, no foul.

Unfortunately just the accusation is enough to get someone fired from their job, lose friends/family and ruin lives. People always remember rape accusations but don't always remember someone being cleared of it. Falsely accusing someone of a horrible crime should carry a penalty regardless of how far along the justice system it goes.

As long as you can prove that someone was lying, then trying to send someone to prison unjustly should be a crime worth going to prison. Rape charge is worse too cause you get the social stigma and put on a list. Bad stuff.

This is absolutely the biggest thing. You have to ask yourself which is worse: raping someone, or lying about being raped? If you lean on one, the other becomes worse. If you come down hard on false rape accusations, honest victims are less likely to seek help for fear of reprisal. However, it is very well known that even the accusation of rape can ruin a person's life, and allowing someone to get away with it is reprehensible.

I agree with @thecatspaw, there is no good solution.

The system is supposed to be innocent until proven guilty; better to let nine rapists go free than wrongly convict one innocent man. While it's a problem that people could get away with crime, the other choice is to convict innocent people.

Does it have to be malicious? but when is making false rape claims not malicious?

This equates to proving the accuser knew she had not been raped.

It's both attempted kidnapping (because the accused would go to jail), and slander. So the punishment should be comparable to a combination of those.
