I always found the idea to be bullshit because it puts so much weight and responsibility on the teacher - if I warn you once about something but then miss something else I'll get in shit... it isn't the instructors responsibility to know about everything that might trouble a student and have to be responsibile for clearly marking it out and warning people.
That way of thinking, it relieves one of personal responsibility completely, you'd have the ability to blame anything you found offensive on the instructor.

Just say "evrything ever we do in this class might offend someone"
And be dont with it.
