I know a Sim City guy made a mod for Cities Skylines.

JSawyer made a mod for FNV, to make the difficulty more in line for what he wanted for hardcore mode. This is a few years after they finished and shipped it.

One of the developers of the Witcher made a rather impressive mod for The Witcher 1 which expanded and improved its combat system, IIRC.
