Hmm yeah if my vinegar was dead, it would take a long time for something else to colonize the wine. I thought 4 weeks would have been more than enough time.

I know some folks might disagree, but there's nothing wrong with buying a mother starter. Amazon has a couple for sale (search for vinegar mother, NOT for acetobacter.)

If the purchased one doesn't work, then blaming preservatives is a pretty safe bet.
