As a physician, I would read this and toss it back at the whale, then tell them the truth. No physician gives two shits about this nonsense and will do what they are trained to do. P.S. that symbol is the caduceus, which is the symbol of thievery. The symbol of medicine is a single snake wrapped around a rod, called the rod of Asclepius. So I would chastise that person double.

I am a registered nurse. If a patient handed this to me I would probably just give a sarcastic smile, then go mock this idiot behind their back in the break room or something. I'd probably also make photocopies of the card and post them around the unit for everyone to laugh at.

If you were my nurse I would love you forever

Mockery is exactly the right response. The only card I can think of that's more annoying is the "I am calling you out...don't play 20 questions with me" one.

Also as a physician, I would just give it back to them and say "I don't think we are a good match for each other. You may be better off looking for another doctor." 10,000 College complaints about bullshit stopped in their tracks right there.

But it's evidence-based!

"Well, so much for the caduceus. Somebody obviously got the wrong symbol for modern medicine–or did they? The caduceus seems to be an appropriate symbol for modern commercial medicine. Of particular relevance are the functions of escorting souls of the dead, wisdom, fertility, commerce, luck, eloquence, cheating and thieving. These have become symbolic of how medicine evolved in the late Twentieth Century." — Luke Van Orden, Where Have All the Healers Gone?: A Doctor's Recovery Journey
I'm not in any way implying that either the fat waste who made this card is using said symbol on purpose, or that modern medicine isn't on the whole a great thing, just that the increased commercialization of medicine into big business is something that should be common knowledge. Whats more profitable, an antibiotic taken for two weeks or an antidepressant taken for two years? We wonder why there has been little headway in antibiotics since the mid 1900's.

"That's a bad infection. Take this antidepressant for two years."
Said no doctor ever.
