200lb in 2 years is almost 2lb/week. Assuming PCOS slowed its BMR from a standard 1400 to (let's be generous) 1200 and then add back on a few to make a TDEE, that brings us to around 1600, assuming a sedentary lifestyle. Now, in order to gain 200 pounds, the fat would need to consume 700,000 surplus calories in two years. That divides down to the best part of 1000 calories extra per day. The fat was eating around 2500 calories/day. That is why it is fat.
