Well, there are some fat males in the Witcher games. Merchants and people like that.

Well, merchants typically had money and slaves. Pretty sure there's always been a select few, of the most privileged, of each generation that have always had more than their adequate portions, in food. Try telling a whale that they're the privileged ones, for having the luxury of making themselves obese, though.

It's because there's no such thing as starvation mode, when you live on subsistence agriculture and more or less starve for 3-4 months a year (winter in Europe) the body burns those fat stores pretty quick.

No, it's set in eastern Europe during the middle ages.

I'm not sure post-apocalyptic is quite right, but it isn't entirely wrong. It's set after an event called "the conjunction of the spheres" where, basically, magic and monsters bled into whatever world The Witcher is set in (Earth maybe?). It definitely caused a lot of chaos, but society still keeps going. More or less.
