If organs floated in the fat an x-ray of an obese person would be insane. All their stuff just floating everywhere instead of looking like a little skeleton in a fat suit. It is easiest to think on terms of surgery. You cut the skin, cut through the fat, cut the layer that holds in the guts and then you can reach the organs. There are also other different organ compartments. Like the heart is in a little bag. The brain is in a little bag. That is why there are different degrees of issues to where the internal bleeding occurs. A vessel bursts in the brain and it fills up the balloon putting deadly pressure on the brain instead of leaking down the spine. Bleeding in the gut is bad but you have more time to get to deal with it.
