My only theory is that they're working in a home that doesn't care for the residents well. The fat at orientation mentioned how her old job was more lax and they got to sit down more. We took a tour around the building and anyone who wasn't working in an office was on their feet, active. God I could see the regret in the orientation lady's eyes.

The fat PSW shouldn't be putting on airs about people wearing scrubs when they "aren't nurses," either, since she isn't one herself.
You don't need to try to one-up her; don't let her pull you into that bullshit (and besides, you win anyway just by virtue of being not a fat cunt like her) because it will only lower you to her level and potentially make you look bad. She's bound to get fired at some point soon-ish anyway if she has such a hard time even pretending to be nice to women more slim and attractive than herself.
It's not like the people you're caring for don't have families, right? Wait until she's a bitch to one of them. Let her jeopardize her job, not yours. Just be as nice to her as you possibly can, and I don't mean fakey-nice. I mean genuinely nice. Make a game of being as polite and friendly as possible, always pretending to think she means whatever she says in the nicest way you can imagine, always acting like you think her shitty attitude is down to a "misunderstanding" or trying to be helpful, always commiserating with her when she tries to insult and one-up you like you think she's trying to be your friend, etc. It makes you look great and makes her look even worse, and people do notice that shit. I've done it before, numerous times; it can be pretty fun, actually, and the best part is they get so frustrated when you won't take the bait or don't "get" their point.
Like, when she says the thing about her test taking five hours and she couldn't even finish it, you could say something like, "Wow, Bertha, that must have been so frustrating! I bet you felt great when you finally got it done though [or will feel great when you finally get it done, whichever is applicable]--did you [will you] do something fun to celebrate?" This gives her nothing to work with. She can't feel "superior" to you because you refuse to feel inferior and seem honestly, innocently pleased for her; she can't feel smug about you being bitchy or jealous because your reply wasn't remotely either of those things but was genuinely sweet and friendly; she can't think you're making some kind of dig at her because there's no way to see that comment that way.
I know this doesn't seem really fun/funny, but I swear to you it will pay off in the long run. It will drive her crazy. Watching her try to come up with ways to be a bitch to you will be hilarious, and her eventual implosion will be even more hilarious. I've done this before, more than once, and each time it ended up being so much fun.
Then come rant about her here, where it's safe. Don't ever do it at work. The better you are, the worse she gets and looks, and the more likely she is to start acting out and get fired. Remember, fats are like bratty children; they can never control themselves for long (if they could, they wouldn't be fat; if they were emotionally stable, they wouldn't be fat; if they were responsible, they wouldn't be fat). This bitch has already proven she's incapable of acting like a professional, so just sit back and wait!

I don't know how things work in America (or Canada, wherever, just not Europe), but I've been left with an extremely negative impression of nursing as a profession. From the people I've met online, it seems like it's not a serious career (like you'd expect from something involving medicine), but an option for washed-up losers that anyone can qualify in. The worst two people I ever met on line were a couple of morbidly obese lesbians, both with severe psychiatric disorders including schizophrenia... one of them was studying to be a nurse.

Speaking specifically for Canadian Nurses, there is two types of nursing:
-4 year bachelors degree program, you are dealing with patients who are in unstable condition. You put in IV's and deal with medications. RN's work mainly at hospitals. Lots of biology and math involved. You need to have good grades to get in. If anything, its the opposite of a washed up loser career. Pays about $30-$50 an hour. You are able to get your masters after completing this program and work experience. A masters in Nursing makes you a nurse practitioner, and you have the power to diagnose patients and prescribe some medications.
-2 year dipolma/associates degree. You are working with patients who need help, but are otherwise in stable condition. Many of these LPN/RPN work in long term care, or hospitals. You still need to have good math skills and a grasp of biology/chemistry. Slightly easier to get into than a 4 year university program. Still pays well at $25-$35/hr.
Then there's PSW's, you don't need to have any prior knowledge in Math/Biology/Chemistry as you are pretty much assisting with daily living activities. Still an important job, but very little responsibility. Honestly this is considered the washed up loser job that anyone qualifies in. If you are in a nursing program, you can be a PSW after completing a year of school.

I work with many wonderful RNs and some crappy ones but on average I think they are well educated (I’m in Canada). A hospital based RN who’s willing to put in the hours can easily make 100+K-year plus the benefits, union protection, etc. NPs do pretty much what primary care providers do. There’s this one ER NP I have a borderline crush on - tall, all muscle, smart as a whip. Our ER docs ask him for advice :-)

It must be PSW they're all talking about then when they say they're in "nursing school" or that they're studying to be a "nurse", because most of them have either only high school education or some unrelated college degree... and they're all complete imbeciles, so there's that. It doesn't surprise me at all that these fats would misrepresent their job to make it seem more prestigious.
