Little column A, little column B, little column C.
A recent study showed that it's stupid that makes the fat more so than fat that makes the stupid, though there are also many showing the effects of fat on the brain. I think the more relevant point here, though, is education and culture. People learn shitty habits and are never taught otherwise and never learn what healthy is. They should be questioning what they were taught, but as I say, stupid makes the fat.
Eating disorders are arguable. I would call what they do disordered eating rather than an eating disorder. It's a subtle difference, but it's the difference between a psychiatric condition and dealing with emotions wrong. You don't keep eating when you're 350lb if you are right of mind and there is a marked difference between what people with eating disorders do and what fats do. Fats are just extreme comfort eaters.
Laziness, definitely. Many know they are fat and what they need to do. They don't because they find it easier to stay fat and deal with how miserable it is than to lose weight. Ultimately, everyone loses weight when that scale tips in favour of losing weight being the easier path. The unfortunate part is how high the tolerance bar is set in some people. You should not be able to live with yourself far beyond a 25 BMI, but people get to 45 BMI and think everything is dandy. I don't get it.
At the end of the day, it doesn't make shit of difference. This is not a serious problem, just having eaten too much. No one cares how or why you made a mess on the kitchen floor. We only care that you clean it up promptly.

Seriously. That last part of the second to last paragraph speaks to my frustration with ginormos. Last year (it was after FPH got axed, go figure), I got to a near 25 BMI and I couldn't stand it. Everything felt shitty, as soon as I felt less flexible and uncomfortable in my clothes I quit my booze bender and got back to my normal self. I could not stand what my body was turning into, and immediately took action and fixed it. If I can give up booze, these lard loaves can give up overeating. It's ridiculous that people bloat up to biggie size and never reach a point where they realize or care about how much of their mobility, health, and looks they've given up!
Drives me crazy. I'm lazy as shit but still manage to stay people-sized, because it isn't that damn hard to control yourself or make lifestyle changes if you're going down the wrong path.
