What do you mean Quality Assurance report? Do you mean a test plan which details what and how you will test the game? I've been a game tester before (for betas) and basically I test every menu item and menu choice, and then try a few other ways to break the system, and I just write that down.

You will also need a good sysinfo tool to detail the specs of your PC even down to 30-40 DLL files that might be in use for sound or video especially. Which is why I wrote my own text editor, and you can specify DLLs to look at when you insert sysinfo into the file.

Just by starting up the game, and going back and forth into windows, then back into the game, change the screen res ingame, and go back to windows and back to the game, I can usually fuck something up pretty good.
