"Showerthought" is a loose term that applies to any thought you might have while carrying out a routine task like showering, driving, or daydreaming.

The main character is based on Barrett and Waters. The band was also really not happy with playing in large stadiums. On the last date of "In The Flesh Tour" Waters got annoyed at some fans at spat at one of them. Gilmore no longer played the encores and would just walk out. The Wall is a culmination of all these events and feelings and the band wanting to isolate themselves from their audience.
From there themes from Barrett and Waters childhood crept into the story line.

It is most certainly about walls we put up to protect ourselves from ridicule when our true beings are shown. That is why The Judge sentences him to be exposed and then tears down the wall.
Syd isolated himself as his schizophrenia worsened and he turned to LSD rather than seek help because he didn't want to be known as the "mentally ill" band member.
The "bleeding hearts and the artists" were Waters and the rest of the band, and the "mad buggers wall" was syd when he isolated himself.
And like @bbartokk said when Waters spat at the fan it made news and that was when his wall came down and his true feelings were seen. All that culminated into the wall. It wasn't just about mental illness, we all put up walls some way or another.
