Carnera was a large man for a heavyweight even by today's standards but positively mammoth by the standards of his day (he remained the biggest heavyweight title holder for many decades after his reign). I don't know if he was champ at the time this photo was taken but he was champion during Mussolini's rule in the 30s.

Looks like the photo was taken in 1933 (Carnera was then 26-27) so it may very well have been when he was champion.
