Exactly, if you think you have trained enough, train more. Train to the point that there can't be ANY mistakes or missed shots. Then train some more. Join shooting competitions and three gun clubs, cause that will make you shoot in more tense situations. Then train more.

I'll get these good groupings then one of the rounds is way off and totally misses the silhouette, and I'm like "damn, how did that get over there?"

Need moar practice.

That's fairly common. You get used to the gun, used to the load, and either a load was off from the factory, or you got so comfortable with your gun that you jerked the trigger instead of pulled it. I get that with my rifle sometimes once I found a load i liked.
