So having had a quick look at the article here I'd like to try and point out why nobody getting HIV does not equal 100% prevention by talking about the different kinds of bias
1) The percentage of people in America with HIV right now is approximately 0.003% (1.2million/318.9million) That means that, in the 2.5 years of the study, each man would have had to have sex with, on average, 333,333 members to be exposed to one HIV positive member. That amounts to 365.1 sexual partners per day. That's an active sex life if I ever saw one.
2) Out of all these sexual partners, every single one of them would have had to have been undertaken without the use of a condom. If we are talking about IV drug use then every single injection (again 365 of them per day) would have had to have been undertaken with a dirty needle. Pretty risky behaviour for somebody who has entered into a trial about HIV prevention one would think?
3) When they have sex with this HIV positive person the person must not be on antiretrovirals (as these can make you have undetectable levels of virus in your blood, making you non infectious). Seems very cavalier for somebody who KNOWS they have HIV to not seek out medical attention. For the sake of argument I'm going to assume that if you knew you were HIV positive you'd make the other person protect themselves, given it's illegal to actively transmit the virus if you know you're infectious.
4) This means that actually, you need to have sex with 6 times the number stated above to get an infection, as only 1 in 6 people who are HIV positive don't know it (so wont be taking measures to prevent transmission)
5) AFTER THAT not every sexual encounter is infectious. Numbers range from between a 1.4% and 0.4% chance of infection with each sexual encounter. So again multiply up the numbers.
There is no statistical evidence generated by this article linking the use of Truvada to a prevention of HIV infection. The numbers just dont add up.

An article was actually posted in news and it was pointed out that this wasn't a double blind study and touched on some other issues. And I concur that this study is a tad flawed (and although I post a lot to medical research and mod it I do think a LOT of medical reserach is a tad flawed) but MR news is MR news and so I posted it.
