Part of the reason is that the "diagnosis" itself is merely descriptive, and while you can call them "disorders" it would be like asking why science has been unable to cure adultery, psychopaths, lying, cheating, stealing, or religion. It's a complex mix of factors that in one person may or may not result in the same outcome, and similarly a diverse set of circumstances may give rise to that outcome in a given individual.

The surgical aspects of it are new since they are a result of advances in medical knowledge and technology, but the idea of a "third sex" that is neither male nor female is actually quite old. That is the part that is a normal human attribute. As for body modification, we've been tattooing, piercing, and amputating (think circumcision) body parts since before we created civilization--it isn't something new.

What's normal or not is defined by society, and those social values change over time. Homosexuality is no longer considered a "disease" or "condition" in the DSM V, and I also wonder about gender identity disorder in the DSM V. While the US law tries to protect children from pedos, pedophilia is "normal" in some cultures, like India and Afghanistan. Even though it's illegal in India, it's still easy to find cases likes this.

Science hasn't been able to figure out how the brain works and so why people differ in sexual preferences. We know what the norm is but not why some vary from it.
