Your hair is almost always growing, but has a built in timer before it kills itself and then grows a new one. It only appears that your hair stops growing at that length.

Yes. The hair follicle would still go through apoptosis at the same time.

It doesn't stop growing. It reaches equilibrium with growth and breakage.

Some people have stronger hair, you can change diet to promote stronger hair, better washing habits(quality over quantity), brushing(better brush, more often), and preventing damage can all lengthen hair.

If hair were made of adamantium yes. However you can do all of the above to prevent mid strand breaks which will increase average length.
