That's not completely true. Meat was abundant, as were eggs. Grains were smaller, more fibrous meaning less of the carbohydrates were available to the body.

Combine that with the fact that the gut biome was more diverse, most carbohydrates would have been consumed by gut bacteria before they had a chance to make an effect on blood glucose. Diabetes is a modern disease. Even potatoes contained a fiber that inhibited the digestion of carbohydrates, today's instant potatoes lack this chemical which exists solely in the skin.
