"I went to the drs about the flu. The Dr then asked me about the gaping infested whole in the side of my face. I said what, are people with gaping infected holes in the side of their face more likely to get a cold then people without gaping infected holes in their face".
It's the same principle....the dr is being good, identify clear issues in addition to the problem presented, before they get worse.

The doctor probably said that fats do get strep more, but the fat in its ears prevented it hearing.

I'll be honest, I've walked out on patients before.
Every single time it was because I had to leave before I punched them in the throat or said something that would get me kicked out of residency, those patients were THAT insufferable.
I'm guessing this doctor was thinking along similar lines.
