There are doctors out there that will give you the side eye if you have strep throat and you're over a certain age, so I'm sure the doctor thought obesity and lack of hygiene was a major cause here

Literally government mandated. Meaninful use attestation requires a shitton of metrics (selected from a list of options). One of the easiest and most common to pick are childhood and adult obesity - where you're basically required to talk to at least 80% of obese patients about weight loss during their visit. Fats may bitch about it because they're too fucking stupid to know, but HHS (and doctors, and actual people) knows that obesity is the #1 killer and #1 driver of medical costs in the US.
