An alternative might be to have an FA protagonist, seemingly winning, I.e. the struggle is real but it's all subtle projections and other people's actions get blamed.
It's miserable but outwardly brave and fierce. Especially fierce online. It could be written in a very narcissistic tone and first person. It blocks things it doesn't want to listen to etc.
Final chapter twist could be an epilogue written by a sibling that's dealing with the FA sudden demise and death (just as it gets to its 15 mins of fame). That epilogue could be funeral costs, and dealing with the digital estate of a pro-obesity fatty with selfie after selfie of delusion. (Rants at doctors etc.) And it would switch to talking about the fat in the third person as a lazy, inept tubblr. Sibling would be a shitlord with a serious case of apathy. The writing style could reflect it too. Shitlord is punchier and common sense. Fatty is bumbling and full of fat logic.
