I have a friend who is BMI 18. She's young and the comments do get to her if only for a moment. It's not shame, more the feeling of being turned upon.

This. So much. It's not feeling ashamed of your body when someone makes a comment, it's feeling like you're hated through no fault of your own. Or, alternatively, feeling like you're hated because you make the effort to take care of yourself. "Thin privilege" doesn't exist because anyone can have it if they put down the fork and work hard enough.
