Have you read the books? It gets mentioned quite a lot that Jon and others give Sam their food because they don't want to finish their portions. And during the vast majority of the story the Watch certainly isn't starving. It's summer/autumn, they have plenty of food and they don't ration it particularly well either.

And Sam got the most sedentary job there is. He refuses to train even after being given direct orders to do so. All he does is read and eat and take care of a 110 year old man who doesn't really need a lot of help. The only times he has been starving was when they fled from the wights and the White Walkers after the fist, and that can't have been too long.

After that, when it gets tough with food at the Wall Sam leaves and goes to Braavos, where he just sits in a room and eat. He went there by boat and the only exercise he got was Gilly riding his fat pink mast.

So surely he shouldn't be as morbidly obese as he still is but he is far from healthy as of the end of AFFC/ADWD. Several characters (including he himself) still call him fat

I agree, his character really should be losing weight. But him being fat at the start was fine for the character, as he had the personality of wanting to be a chef - I cant remember if he said that, but that was my immediate thought of the character. That desire is in the "I have no idea how hard a job that actually is" phase, though.

Sam wanted to be a maester (sort of a Medieval doctor/teacher/advisor) but his dad wouldn't let him so he was forced to go to the Wall. The Watch already had a cook, so I don't think he had any ambitions to replace him
