Not only do they have an insatiable appetite, they have a really low IQ to go with it. Even lower than normal for a tubby.

These are sad, legitimate cases of conditions that are out of the patients' control. HAES, a lack of effort to curb diet or get off the sofa are not legitimate reasons to let yourself get dangerously fat.

Praeder-Willi syndrome is a legitimate genetic condition (a piece of chromosome 15 is missing or inactive). They have a specific facial appearance (very much like people with Downs) and have a bunch of other problems like incomplete sexual development and mental disability.
Once my flatmate self-diagnosed herself with Praeder-Willi, trying to use it as a kondishun for why she was enormous and I couldn't stop laughing for days. Any reason will do for these people.

....she actually tried to use Praeder-Willi as an excuse for her whaledom? And expected to be believed? Really?
Well there's my headache for the day. Jesus, the stupidity.

It is, to some extent, a legitimate condition - as the fatties may be mentally incapable of taking care of themself, and may be incapable of knowing when to stop eating.
I would liken this to a person in a coma who is overfed by the hospital. The person's conscious mind doesn't really have much/any control over it.
However, the No Sympathy rule applies across-the-board, whether it's a legitimate condition or not.

Although this definitely makes life a lot harder, due to low IQ, incredibly high tolerance to pain and a truly insatiable appetite, and it is actually caused by genetics, it's not the end of the world:

So no, generally it isn't.
