Mostly in my native language (portuguese). I think in english or german when I am having a conversation in those languages, or reading/writing in them. I cant have two different languages in my mind at the same time. Even if is just the TV at the background, I tend to switch to the same language as it.
People who can do live translation baffles me. The quick switching back and forth between languages is impossible to me.

I worked with a Brazilian woman who said that her internal monologue switched completely to English after 10 years of living in he US.

I think in the language that I'm currently using. If I'm talking to people in German, then my thoughts are German. If I talk to Americans, my thoughts are in English. My thoughts also change to whichever language that I'm currently reading, so for example on voat, my thoughts are in English and if I read a German news site they switch to German.
I still remember when I learned my third language, French, in the beginning it felt like I was thinking like a translator; thinking in English or German and then translating words to French, but once I started getting better at the language, I actually started thinking in French.

It depends. My mother tongue is Arabic but I'm a fluent speaker of English and French. Most of the time I think in Arabic. However sometimes I'm playing games IRL, I think like everyone else. When for instance you're navigating a new premise, or imagining a scenario, that's when I switch to whichever language represents the character I'm imagining myself. I don't know if it makes sense.
