i agree that kid was clueless. going through all the proper steps may be a pain in the ass, but its important. but some people arent warned about the dangers of their job. OSHA 30 is boring as hell but it teaches you to value your life and how to value your life.

no job is worth your life, if theyre not paying you to have the time to do it right, dont do it at all. i couldnt tell how the fire got started. im not exactly sure what this contraption is. almost seemed like it wasnt locked-out-tagged-out. do you know what exactly happened here?

Spontaneous combustion, causing flash explosion, is the direct cause of the accident.

I had to single-frame. It looks like the blast accelerated him up and then he hit other stuff and got knocked to bits. It's amazing how much force that had given that he was only leaning slightly over the hole.
