(Didn't we see something like that here [quite] a while ago? Ohhh, no, it was about something else, but it was the same principal: make someone who thinks they know better sign a waiver acknowledging that they are putting their health at great risk [in the example I'm thinking of, it was their baby's health; it might have been elsewhere that I saw it] by refusing to do X or Y.)
I would love to see a form like that:
"I am refusing to be weighed or to discuss my weight today. I understand that by refusing to provide this vital medical information, I am limiting my doctor's ability to understand the full picture of my health, and by refusing to discuss or hear advice related to my weight, I am limiting my doctor's ability to properly treat my problem [or recommend proper treatment]. I understand that refusing to discuss or hear information related to my weight limits my own knowledge about my health and my risks for Type II Diabetes, heart disease, lung problems, skin infections and bacterium, joint and back problems, kidney and liver issues, foot problems, bone problems, and early death in general, and, if I already have any of those conditions, my refusal to discuss my weight or consider weight loss limits my doctor's ability to treat those issues, thereby increasing my risk of early death. I hereby agree that my doctor will not be held liable should any obesity-related illness, disease, or problem cause my early death or simply limit my quality of life. I hereby certify that I am taking charge of my own health and I believe I know my body better than my doctor and my medical knowledge is more extensive than that of my doctor, so should any of those illnesses occur, limit or destroy my quality of life, or cause my early death, the only person liable for that is myself."

Refusing to be weighed does the exact opposite of what this lardbrain thinks and just serves to focus the doctor's attention on its fatness, plus shows that they're touchy and uncooperative, and looking for pampering instead of actual solutions.

A medical doctorate is not a cushy degree no matter what school you went to or what kind of "privelages" you've had in life. It's one of the most difficult degrees to complete and puts you about 200K in debt. Most doctors aren't even rich anymore outside some specialties since the cost of malpractice insurance has been skyrocketing alongside tuition costs. Its extremely hard work and a monumental achievement and it honestly disgusts me that these fat retards believe that Instagram toilet philosophers know better than people who have devoted their lives to the study of medicine at great personal cost. They're trying to save your worthless life, fatso!

Yes, that's always my favorite thing about the "weight refusal" crowd. You really think without a number on a scale, the doctor is just mystified about your body size ad weight? Are you visiting a blind and deaf doctor who never touches you? Because believe it, pig, your doctor not only knows that you're hugely obese, s/he probably has a pretty good idea of exactly how much you weigh. S/he could probably guess ad be accurate to within 30-50 lbs. All you're accomplishing is proving to everyone that you're an asshole baby know-it-all.

You dumb fatfuck! Not being weighed is thin privilege? Listen up, fatso - I WANT my doc to check all the basics when I go for a visit. I pride myself on how well I know my own body but shit can creep up on you sometimes, and the doc is the person most likely to give you the “how long has xxxxx been xxxx...” that I may miss, thus, potentially, saving my bacon!
What I don’t want is health professionals failing to ask me questions because some fuckwit might be offended.
