The fatty is basically pulling a 7 year old trying to get out of school by complaining that his ovaries are having a heart attack.
Like most words that end with ~ism, ~ist and ~phobia, a lot of people aren't actually familiar with what anorexia really is. To most people it's just a word that means dangerous disease where you don't eat enough.
All in all it's a manipulation tactic... You wouldn't tell this that maybe she doesn't need that double bacon cheeseburger (unless she's on her fifth and is just gonna go hug the porcelain prince after.) and should maybe eat a small salad instead.

Because they bastardized and manipulated the word to fit their narratives like how they transformed privileged, oppressed, racism, nazi. And so on.
They assume they are anorexic because if their attitudes toward their hideousness even though the DSM V explicitly states the requirements that allow for its proper diagnosis.

I have never had an eating disorder but there's a couple of reasons I can think of - they don't seem to understand the difference between moderate calorie control and becoming obsessed over calories. To them any form of calorie counting or calorie restriction is the exact same thing as having a panic attack over eating one walnut. Two, they have warped body image. Just like a person with anorexia might feel fat at bmi 18-20 fats will look at someone of the same weight and see an emaciated skeleton. So if they were ever somewhat in shape at any point in their lives they claim they used to be too thin.
