I saw a post a few months ago that a US army doctor that treated the Germans waiting trial at Nuremberg say that the majority of the prisoners had their testicles so badly damaged during interrogation some had to have them remove.

Blunt force trauma.
Doesn't take very much force to damage the skull or brain, and unless you're hitting the face or breaking skin, it's probably not going to leave any visible marks either.
Look up the "breaking wheel", a medieval torture/execution method, for context. You can break nearly every bone in someone's body without ever breaking the skin.
