User error. Probably an intern was tasked with reviewing all of their canned preset reports for breaking news and one somehow got put up.

Like do they have preset disasters and someone hit the wrong button?

Basically news stations have a list of preset headlines to run in case of breaking news. It can be anywhere from a celebrity dying to Russia invading the Ukraine. If I had to guess what happened is someone was reviewing them before the broadcast, as they need to keep the list updated and know which ones they have available, and they accidentally ran it.
