Its a temporary cavity in gel. While that gel has the same density as flesh, bullets don't affect flesh in that exact way. And it is generally considered that the temporary wound cavity made by pistol calibers don't actually contribute to real wounding, but only when you are pushing 3000fps with rifle rounds and causing GIANT temporary wound cavities. So its that thin hole in the middle that is killing you, not the big bubble at the start. That cavity just seals in a split second, while the central wound suffers severe bleeding.

I don't know how louder it really is, but i once shot an AK indoors without earpro as a kid and it wasn't that big of a deal as far as i remember. You are fighting for your life, who gives a shit, im not gonna pick calibers based on dB.
