You know there was a tough choice made for this photo. Either she has to stand for the photo like this or she has to climb the stairs of the train to sit for the photo. Can you imagine the gears in her head turning trying to make that call?

Their lives and mental/emotional behaviors are disasters. They struggle to attain normalcy, often only achieve semblance. I see their fat as their scar tissue. They need therapy equivalent of plastic surgery concurrent with health advisor. These are huge ventures not easily embarked.
