Use toothpaste that doesn't foam. Trust me on this. I've known a couple of people who had this problem and it was the foam every time. Just try baking soda for one brushing. Salty as fuck but no foam. If that fixes the problem, you can either stick with baking soda or you can get special toothpastes that don't foam.

Stick your tongue out further so you aren't hitting the back of your throat when you brush it. To get the very back of your throat you should just gargle some mouth wash.
