I suppose it would be a pain in the ass for police, firefighters and so forth if they had to juggle between ten different ambulance services based on distance, cost, and the personal preferences of the dying.

Every area is different I guess. My county has 50+ companies(most of them corrupt or committing fraud) but you as the patient have the right to transported by any company you want regardless of any contract an ambulance company may have with the city, to any hospital you want in the area. Now that being said if you have Medicare/Medicaid they will only pay for you to be transported to the closest facility. You would pay the difference, unless you need specialized care not provided at the closest facility. Usually insurances follow suit with Medicare guidelines.

My area used to have two ambulance companies. One of them paid off dispatch and politicians to make sure they got all the calls. Now we only have one ambulance company.

Not anymore. This was years ago. The politicians in office at the time have long since left office and are living the cushy life. The dispatcher responsible died on the way to the hospital after an accident. Weird how that works.
They tried burying this real hard, the only reason I found out was because my cousin worked for the company that left town. The one that paid off everyone tried to scoop up everyone working for the company that left, but my cousin refused to work for someone with that kind of ethics. A year later, the company that stayed massively slashed paychecks.

because i think id rather have a trained medic pick me up in an ambulance full of drugs and shit than some bum trying to turn a quick buck.
I meant why don't we have competition between two or more ambulance companies in the same area/city. I was using the ride sharing companies as an analogy.
