The article could use some edits. I've done the first two paragraphs:

With extremely rare exceptions, obesity is always, always a choice. The patient in the article chose to become 500 pounds, every single forkful of food that she didn't have to, but did, eat.
"Help Doctor, it hurts if I do this", needs to be followed by "then don't do it". Don't want to die of obesity at 40? Don't choose to become obese.

Are you suggesting that individuals have free will, choose the actions in their life and experience the consequences of their choices? Maybe smokers get lung cancer by chance and fat people just happen to die years earlier.
People are selfish. They want everything now and when disease appears they want a cure. "I have hypertension, give me a pill. I won't change anything, I want drugs. Why should I change anything?". The best thing about obesity is that it kills. It objectively is a trait that will die out we are watching evolutionary history unfold; enjoy the theatre.

Obese people's bodies are failing obese people. Medicine is better than it has ever been. Yeah, fuck all this advancement, if it's not able to save some 800lb house cow from eating itself to death, why bother? Let's go back to dying from splinters and diarrhea.

Those assholes got it backwards. Medical care isn’t "failing obese people”--it’s telling them things they don’t want and refuse to hear. The truth is that obese people are intentionally failing to be receptive to effective health care.

Doctor: Your health issues are because you are fat. And you are fat because you eat too much food. You need to eat less food, which will cause your weight to decrease, which will alleviate most of your health issues.
