The red pill was when Neo went looking for freedom and self actualization down the rabbit hole in Matrix I. The black pill was Episode II when Neo realized the rabbit hole itself was just another layer of control. Remember when the Black Oracle offered him candy. Her candy was an homage to the pill Morpheus gave Neo and a mockery of the idea of the original pill choice. Ultimately, the only true freedom is total destruction of everything- a bitter pill which Neo chose not to take and instead save both the systems of control and the deluded fools seeking freedom.
And yet, choice remains...

If the red pill is the realistic appraisal of reality, the black pill is the nihilistic one.
A person who has been black pilled has been bombarded with disheartening information about an issue that, while perhaps - but not always, sometimes the black pill is the intended treatment - intended to red pill that person, has instead convinced the person that things are so bad, that the rot has become irreversible, and it will only ever get worse. Without any counter treatment, the black pill generally leads to tendencies that are destructive to others and oneself.
